SIGNING RULES

How it is signed in the United Kingdom

The rules for an official government form depend on where it is signed. Laws change, so check the sources before anyone signs.

LP1F and LP1H

Official form

A lasting power of attorney lets a person aged 18 or over (the donor) choose attorneys for financial decisions (form LP1F) or health and care decisions (form LP1H). The donor, a certificate provider, each attorney and the witnesses sign the same paper original in ink, and the LPA must be registered with the Office of the Public Guardian before it can be used.

Steps

  1. Download form LP1F (financial decisions) or LP1H (health and care decisions) from GOV.UK.
  2. Choose who fills each part: the donor, the certificate provider and each attorney.
  3. Send it so each person fills in their part, then print one original for everyone to sign in ink.

THE RIGHT TEMPLATE

Where it fits

For England and Wales only. Scotland and Northern Ireland have their own powers of attorney. It can also be made with the OPG online service at www.lastingpowerofattorney.service.gov.uk, which prints the forms. Sign does not rewrite the forms.

A CAREFUL CHECK

Before you rely on it

Original Centilio wording, not legal advice. GOV.UK says everyone must sign the same original and cannot sign copies or use digital signatures, so the form is printed and signed in ink, and the LPA cannot be used until the OPG registers it, for a fee. It does not cover enduring powers of attorney, general powers of attorney, Scotland or Northern Ireland. See a solicitor if the donor's capacity is in doubt or their affairs are complex.
