THE RIGHT TEMPLATE

Where it fits

For an adult in India changing their own name, as the first step before the newspaper advertisement and gazette notification. Use the one and the same person affidavit when you keep your name and only its spelling or initials differ across documents.

A CAREFUL CHECK

Before you rely on it

Original Centilio wording, not legal advice. An affidavit for an adult in India changing their own name, affirmed before a notary under the Notaries Act 1952 or a person empowered under section 3 of the Oaths Act 1969, on stamp paper of the value the state sets. The affidavit alone does not change any record. A newspaper advertisement and a notification in the Gazette of India or the state gazette are usually needed after it, and each office (Aadhaar, PAN, passport, bank, school board) then has its own process. A married woman taking her husband's name may need only her marriage certificate for some offices. It does not cover a child's change of name, which a parent or guardian makes. Ask a lawyer when the change touches property, inheritance or a pending case.
